From modest beginnings 25 years ago, Mecklenburg Open Door has grown rapidly into one of the county’s largest mental health agencies.

With a budget of $6 million, it currently serves more than 500 mentally ill clients, helping many of them get the housing and services they need.

Now, however, on the heels of a federal review that found a host of problems, the nonprofit’s future is in jeopardy.
