HICKORY – The N.C. Division of Health Service Regulation Adult Care Licensure Section conducted a complaint investigation of Walden House on July 9, 10 and 13. The Catawba County Department of Social Services initiated the investigation July 9. The state found seven deficiencies. 1. The facility must adhere to the care plan established for each resident under the care a mental-health provider, developmental disabilities or substance abuse services
